Supported
Living


Rekindle Supported Living strives to help individuals with complex needs associated with intellectual or developmental disabilities. Rekindle Living understands that some individuals with intellectual disabilities might have some behavior health challenges which is why our personalized approach focuses on the individual in order to understand their needs. Our personalized approach to independence is person centered aimed at empowering our individuals in order to help them live and become more involved in their communities, which can be through community engagement and community activities with their peers. By working with the individuals and their families to develop the individuals Services plans (ISP), Rekindle Living go over the individuals goals and outcomes and support needed. All of our staff receive 8 hours of providers training and additional on site training with the individuals to make sure they are the right fit for the individuals. Staff help the individuals with daily activities which may include but not limited to Cooking, Housekeeping, Hygiene, Grocery shopping, Money management community activities and conflict resolution.

Living


Rekindle Shared Living match individuals with intellectual and developmental disabilities with compatible hosts to live in private homes with passionate caregivers to help the individuals with daily activities. These hosts are more than caregivers because they serve as advocates, friends, teachers and family. Our Rekindle hosts are passionate about delivering personalized services to help the individuals live fulfilled life, while also helping them with learning new skills, socialization with their peers, household chores, maintaining their independence, community engagement and exploring new interests. Rekindle Shared Living also support the individuals with around the clock support, advocating for the individuals, behavior support, medication administration and oversight, evaluation of programs, family support and life skills.

Respite


Services provided to an individual unable to care for himself or herself furnished on a short-term basis because of the absence or need for relief of those persons routinely providing the care. Community respite shall only be provided outside of an individual's home.

Transportation that is used by individuals enrolled in individual options, level one, and self-empowered life funding waivers to get to and/or from a place of employment or to access adult day support, career planning, group employment support, individual employment support, and/or vocational habilitation.

Management


Services that provide assistance to individuals who need support managing personal and financial affairs, including training to assist an individual to acquire, retain, or improve related skills. The services meet a continuum of individualized needs, from organizing and keeping track of financial records and health insurance documentation, to assisting with bill-paying and maintaining bank accounts
